Yess technique was first applied since 1997 and the inside out technique onlygot 80% success rate. The PELD begin applying for recurrent and far lateral disc cases since 1999, infective spondylodiscitis cases since 2000, lateral stenosis and failed back syndrome since 2003. Since 2002, targeted fragmentectomy technique was applied and the success rate in contained and noncontained disc case can reach 92%. The recurrent rate in contained disc around 3.2 % and since late 2005, spinal scopic shaving and bur system were applied and can deal with lateal stenosis , Gr I spondylolisthesis and failed back syndrome cases with sciatica problem in aging patients. This is very exciting thing due to the aging spinal problems can be manage by spinal scope system under local anesthesia. Using this technique, the post element of the spine do not damage and back functional performance do not influence during operation. There also have the advantage of lower complication rate, rapid recovery,
o need blood transfusion and no general anesthesia sequeala . The success rateincrease to 92-95% which more close to convential surgery. We believe this technique will be the first choice for many patient who are candidate for this tenichque. It also will be apply in spinal fusion, disc replacement, autogenous chondrocyte cell transplantation in the near future.
